Financial support
The Foundation is able to assist patient members who are facing financial difficulties as a result of their medical conditions.
Assistance may be in the form of vouchers to help with hospital expenses for long term in-patient situations, specific medical equipment /medicines, adrenaline Epipens or other small grants towards relevant medical expenses.
All inquiries must be made through our formal needs assessment process.
A family grant request form may be downloaded from our Resources section and completed directly by the patient member (or guardian), and submitted to our Support coordinator . The patient member may be contacted and an interview time arranged to discuss the application and complete a formal needs assessment evaluation form which the patient member (or guardian) may be asked to sign off.
Support staff will do their best to help the patient member identify any entitlements from other agencies, and to present any specific requests for family grants to the IDFNZ Board for consideration.
All qualifying applications will be considered by the Board subject to funding constraints. The Board decision is final and binding.
