All inquiries must be made through our formal needs assessment process.

A family grant request form may be downloaded from our Resources section and completed directly by the patient member (or guardian), and submitted to our Support coordinator . The patient member may be contacted  and an interview time arranged to discuss the application and complete a formal needs assessment evaluation form which the patient member (or guardian) will be asked to sign off as true and correct.

Support staff will do their best to help the patient member identify any entitlements from other agencies, and to present any specific requests for family grants to the Foundation Board for consideration.

All qualifying applications will be considered by the Board subject to funding constraints. The Board decision is final and binding.
